Developing program

In development

Hospitals & transfers.
Emergency care needs a connected pathway.

A stronger network around psychiatric emergency entry, transfers, hospitalization, discharge, and the next appropriate level of care.

The program in context

When hospitalization is needed, the pathway cannot be another barrier.

Some people genuinely need emergency psychiatric care or hospitalization. Yet fear of the setting, cost, insurance restrictions, cultural misunderstanding, and uncertainty about what comes after discharge can keep people from entering when they should.

The same disconnection can follow a person out of the hospital. Without a dependable handoff, appropriate outpatient support, and communication with trusted relationships when permitted, someone may leave stabilized for the moment but more vulnerable in the days that follow.

Thryve is cultivating relationships with hospitals, CPEP and psychiatric emergency programs, higher levels of care, discharge teams, and clinical organizations to strengthen the entire route—not only one end of it.

01

Emergency entry

A clearer route into appropriate psychiatric emergency evaluation and hospitalization when genuinely needed.

02

Transfers & placement

Stronger relationships that help identify and reach the setting suited to the person’s clinical need.

03

Discharge & next level

A defined connection from hospital care into responsive outpatient, intensive, residential, or community support.

The model taking shape

From an identified gap
to a working pathway.

01

Build trusted relationships

Create working connections with hospitals, emergency programs, clinical teams, and community referral partners.

02

Clarify the handoff

Make ownership and the next step visible when a person enters, transfers, or leaves a higher level of care.

03

Reconnect after discharge

Link the person to the appropriate next level while keeping permitted family, provider, and community supports informed.
